Sand Canyon Urgent Care (“we,” “our,” or “us”) is committed to protecting your privacy and complying with the Health Insurance Portability and Accountability Act (HIPAA) and California’s Confidentiality of Medical Information Act (CMIA). 5. Security Measures

Despite these measures, no system is 100% secure. We encourage users to protect their credentials and report any security concerns.
6. Patient Rights Under HIPAA
You have the right to:

- Request an Amendment: Ask for corrections to inaccurate health information.
- Receive an Accounting of Disclosures: See a list of parties who have accessed your PHI.
- Request Restrictions: Limit how your information is shared (though we are not required to comply in all cases).
- Opt-Out of Marketing Communications: Unsubscribe from promotional emails.
- File a Complaint: Report violations to our Privacy Officer or the U.S. Department of Health and Human Services (HHS).

7. Special Notice to California Residents
Under the California Consumer Privacy Act (CCPA) and Confidentiality of Medical Information Act (CMIA), California residents have the right to:

- Request deletion of non-medical personal data (Note: PHI is exempt from deletion requests under HIPAA and CMIA)
- Opt-out of data sharing for advertising purposes

8. Breach Notification Policy
In compliance with HIPAA’s Breach Notification Rule, we will notify affected individuals of any data breaches involving PHI within 60 days of discovery. If a breach affects 500 or more individuals, we will notify the U.S. Department of Health and Human Services (HHS) and the media, as required by law.
9. Children’s Privacy

10. Changes to Our Privacy Policy
