A Doctor’s Guide to Chest Pain Symptoms: What They Mean and What to Do Next

Your heart skips a beat. You feel chest tightness. Panic sets in. You wonder, “Is this a heart attack, or something less serious?” When experiencing chest pain symptoms or heart pain symptoms, knowing when and where to seek care can save your life (and save you money).

Chest pain is one of the most common reasons people visit an emergency room, but not all chest pain is a cardiac emergency. This guide from the Sand Canyon Urgent Care clinical team will help you recognize your symptoms, understand what might be causing them, and decide the right level of care for treatment.

Emergency Room: Chest Pain Symptoms That Require 911 🚨

The ER is for life-threatening cardiac emergencies. Call 911 immediately and do not drive yourself,  if you experience any of the following:

Urgent Care: Chest Pain Symptoms That Can Be Evaluated Here

Sand Canyon Urgent Care🩺 Cash pay visit with EKG: $250

Emergency Room 🏥Average cost: $2,500–$5,000+

While life-threatening emergencies require the ER, urgent care is an appropriate and cost-effective choice for non-critical chest pain concerns.

Urgent care is a smart first step when symptoms are:

⚠️ Important Note on Women’s Heart Attack Symptoms
Heart disease is the #1 killer of women in the United States, yet women are more likely to delay seeking care because their symptoms don’t match the “classic” heart attack image. Symptoms like unexplained fatigue, nausea, back or jaw pain, and shortness of breath deserve the same urgency as crushing chest pain. When in doubt, seek evaluation.

Sand Canyon Urgent Care is a trusted first point of contact for chest pain evaluation. Our providers can assess your symptoms, perform an EKG, review your medical history, and determine whether further care or specialist follow-up is needed, all in under an hour, with no appointment required.

The 6 Most Common Chest Pain Causes

Chest pain symptoms can originate from several different body systems, not just the heart. That’s why a proper evaluation is so important. Common causes include:

❤️Cardiac
Arrhythmia, angina, heart attack
🍽️Gastrointestinal
Heartburn, GERD, stomach ulcer
🫁Respiratory
Pneumonia, pleuritis, pulmonary embolism
🦴Musculoskeletal
Costochondritis, muscle strain, rib injury
🧠Anxiety / Panic
Panic attack, stress-induced chest tightness
🔍Other
Thyroid issues, medication side effects

The doctors at Sand Canyon Urgent Care can determine in under an hour whether an ER visit is necessary, saving you time and money.

What Is Costochondritis? 🦴

To understand this common cause of chest pain, we can break down the word: costo (rib) + chondr (cartilage) + itis (inflammation). While the symptoms often mimic cardiac or heart-related concerns, costochondritis is a non-life-threatening condition. It occurs when the cartilage, or flexible connective tissue connecting a rib to the breastbone (sternum), becomes inflamed. It’s a very common reason patients seek urgent care evaluation and is not life-threatening.

💡 Pro-Tip: The “Click” Test
Patients can often self-identify costochondritis if their chest pain is reproducible, meaning they can recreate the pain themselves. If pressing on the specific spot where the rib meets the breastbone makes the pain sharper, that is a classic sign of inflammation rather than a heart issue. Try pressing gently along your sternum. If you find a spot that replicates your chest pain, mention it to your provider.

How Doctors Assess Your Risk 📊

Using the Marburg Heart Score

To help determine the right level of care, clinicians use a validated clinical tool called the Marburg Heart Score (MHS). This tool assesses your risk of Coronary Heart Disease (CHD), the leading cause of heart attacks, by evaluating your age, medical history, and specific symptoms.

You can calculate your Marburg Heart Score online at MDCalc:

Score 3+

Go to the ER immediately for a higher level of cardiac evaluation.

Score 1-2

Electrocardiogram Tests

An EKG (electrocardiogram) is a quick, painless test that records your heart’s electrical activity. Small sticky patches called electrodes are placed on your chest, arms, and legs, and connected to a machine by wires. The machine records the natural electrical impulses that coordinate your heart’s contractions. No electricity is sent into your body.

The test takes only a few minutes, and your provider can review the results right away. An EKG can show your heart rate, heart rhythm, irregular heartbeats, and signs of a current or past heart attack.

Can an EKG Detect a Heart Attack?

Yes, an EKG is one of the first tests performed when someone presents with heart attack symptoms. EKGs can detect heart attacks as they happen by identifying abnormalities in the heart’s electrical patterns. They can also detect previous heart attacks, including “silent” heart attacks that had no noticeable symptoms, though with moderate accuracy.

Understanding EKG Limitations

It’s important to understand that an EKG is a “snapshot” in time. It shows your heart’s activity only during those specific seconds. This means:

If the EKG is normal: This doesn’t definitively rule out a blockage or intermittent arrhythmia. Providers often recommend follow-up testing like a Holter monitor (24 – 48 hour monitoring) or a stress test to catch symptoms that don’t occur during the office visit.

If the EKG is abnormal: We will coordinate immediate ER transport for comprehensive cardiac evaluation.

Chest Pain Symptoms: Frequently Asked Questions

💬 Questions to Ask Your Provider

When describing heart-related symptoms, be specific about the sensation, timing, and duration. Ask your provider:

“What symptoms would mean I should go to the ER immediately?”

“Is my EKG rhythm normal? Are there any PVCs, PACs, or signs of atrial fibrillation?”

“Could this be related to my electrolytes, thyroid, or other underlying conditions?”

“What triggers should I watch for? Dehydration, stress, caffeine?”

“Do I need a Holter monitor to catch symptoms that don’t occur during the office visit?”

Can anxiety cause chest pain?

Yes. Anxiety and panic attacks are among the most common non-cardiac causes of chest pain. Stress-induced chest tightness can feel alarmingly similar to cardiac pain. An EKG and clinical evaluation can help rule out a cardiac cause and give you peace of mind.

Can heartburn feel like a heart attack?

Absolutely. Acid reflux and GERD can produce a burning sensation in the chest that is easily mistaken for cardiac chest pain. The location and timing of the pain (often after eating) can provide important clues, but a clinical evaluation is the only way to be certain.

What does a heart attack feel like in women?

Women often experience heart attacks differently than the “classic” crushing chest pain. Common symptoms in women include unusual fatigue, shortness of breath, nausea, dizziness, and back or jaw pain — sometimes without any chest pain at all. If you experience these symptoms, especially in combination, seek care immediately.

Is it safe to drive myself to urgent care with chest pain?

If you have severe, crushing chest pain or any of the emergency symptoms listed above, call 911. Do not drive yourself. For mild, stable symptoms that are not worsening, it may be appropriate to drive to urgent care. When in doubt, call us and we can help you decide.

Keeping Track At Home

In addition to monitoring your symptoms, if you or a loved one has a history of chest pain, cardiac problems, or hypertension, at-home monitoring devices can help provide valuable clarity between clinical visits. Tools worth considering that can be purchased online or at your local drug store include:

  •  a pulse oximeter to measure blood oxygen levels,
  • a blood pressure monitor to track cardiovascular readings from the comfort of home,
  • and an at-home EKG monitor to detect irregularities in heart rhythm that may otherwise go unnoticed.
